How the system works
CAJA (the Caja Costarricense de Seguro Social) provides universal public coverage; enrollment requires temporary or permanent residency, not tourist status. Private hospitals (CIMA, Clínica Bíblica, Católica) operate alongside it, and coverage is mandatory for residents.
What expats actually use
Residents enroll in CAJA and pay a percentage of declared income; copays are minimal and prescriptions are largely covered. To skip public wait times, many add private insurance (~USD 200–400/mo) or pay out of pocket for specialists.
Quality and access
CAJA quality is high — life expectancy sits near 80–81 — but non-emergency public waits can run 2–4 months, and the Lancet healthcare-access index (69) sits below the US and Europe. San José and the Central Valley have the best resources; rural and Caribbean areas see delays.
The private market
Private hospitals serve expats and wealthier locals with minimal waits and direct billing. Out-of-pocket private consults run roughly USD 100–200; dental and vision are commonly private.
