How the system works
The Servizio Sanitario Nazionale (SSN) provides universal coverage, ~73.9% publicly funded and decentralized across 20 regions. All legal residents are automatically covered for primary care, hospitalization, prescriptions, and mental health; private insurance (~10% of the market) supplements it.
What expats actually use
Most enroll in the SSN via their regional provider — primary care is free or low-cost, prescriptions are capped. To skip public wait times, many pay out of pocket or add private insurance for specialist access.
Quality and access
Hospitals in Milan, Rome, and Florence are modern and internationally accredited; emergency care is world-class. Regional variation is significant — southern regions see longer waits — and rural access is thinner. Maternal care rates excellent.
The private market
Private insurance (~€80–150/mo) buys faster specialist access; out-of-pocket private consults run ~€40–80 with direct billing. Some expats use private providers entirely to sidestep bureaucracy.
