How the system works
Thailand's public Universal Coverage Scheme (UCS) covers about 99% of Thais through taxes, but it's built for citizens and permanent residents. Expats live in the private sector, anchored by JCI-accredited hospitals like Bumrungrad and Bangkok Hospital, paying via insurance or cash.
What expats actually use
Most buy expat health insurance that meets their visa's minimum (the O-A requires inpatient cover of at least 400,000 THB). GP and specialist visits are paid by cash or card, and pharmacies — well-stocked and everywhere — handle routine needs without a prescription hassle.
Quality and access
Urban hospitals are world-class and English-fluent, with specialist waits often under a week; the WHO has praised the system's efficiency. Rural clinics are basic, and the urban–rural gap is significant — the Lancet healthcare-access index rates Thailand mid-pack (67).
The private market
Private clinics handle consults, dental, and vision on a straightforward cash basis, and international hospital chains sell annual check-up and wellness packages. Many expats use private providers exclusively and simply carry insurance for anything major.
