How the system works
The Serviço Nacional de Saúde (SNS) is universal and tax-funded (~9% of GDP), covering all legal residents after about 120 days. Emergency care is immediate; routine care runs through a family doctor at a centro de saúde, and specialists need a referral. Mental health services exist but are underfunded. Non-urgent waits run 2–6 months for specialist appointments and 3–12 months for surgery.
What expats actually use
You register (free) at a local centro de saúde, use hospital urgência for emergencies, and lean on the SNS for routine care — expecting waits. Private consultations (€50–€100) are widely available and fast (same-week), so many expats use private care for specialists and pay out of pocket. Medications are cheap; dental is usually private.
Quality and access
Public quality is good in Lisbon and Porto and adequate but less specialized in rural areas. Emergency response is reliable, maternal and child health are strong, and life expectancy sits above the OECD average. Private hospitals (CUF, Lusíada, Hospital da Luz) match high-tier standards. Long waits are the main drawback.
The private market
Private care is widely available and affordable, with same-day to next-week appointments and transparent pricing. Insurance exists, but most expats simply pay out of pocket — it's faster and simpler. Mental-health providers concentrate in urban centers, and international insurance covers private care.
